Cox CARE Mobile Forms

The Cox CARE Mobile services the Dallas County School District perioidically. This service offers free basic health services for children from age 2 months through the age of 18 by a nurse practitioner.  The mission is to provide access to health care for children who have no insurance, do not have a primary care physician or whose parents cannot afford to pay for necessary services.  You can click the Pop Out button in the upper right hand corner of the consent form below, to print the document and complete prior to services.
